Joining The Practice

You can register as a new patient if you live in Furnace Green, Southgate and parts of Three Bridges. You can check to see if you live in our practice boundary via the interactive map opposite.

We have a patient leaflet available for new patients. It explains how our appointment system works, who are staff are, and includes much more useful information. You can download it here, or ask at reception for a paper copy.

If you are using our paper registration forms, you can come into the surgery to fill out them, or you can download them from the links above, complete them and email them to sxicb-wsx.furnacegreen-reception@nhs.net

You will need to bring in or email:

  • the completed registration form
  • your allocation letter
  • a copy of your photo ID, such as a driving licence or passport if you have one
  • proof of address if you have one

We are a Safe Surgery for everyone in our practice area. We might ask for ID or proof of address. But if you don’t have any and you live in our practice area, you can still register with us.

Everyone living in England has the right to free care from a GP.

Ask reception for an interpreter if you find it difficult to communicate in English.

Our receptionists won't ask you about your immigration status.

If you are worried about giving us your address, please let us know. Your information is safe with us.

Once you have submitted your registration form

Your registration will be processed in about one week. You will receive a text message letting you know when your registration has been completed. In the meantime, if you need medical help, you can contact your previous surgery for advice or visit the Urgent Treatment Centre at Crawley Hospital.

New Patient Health Check

As a new patient, you may be required to have a new patient health check if you are aged 40 or over and do not already have a diagnosis of high blood pressure, heart disease, diabetes or kidney disease.  This will be a general check up, normally including a urine sample to test for underlying medical issues like diabetes.

Temporary Patient Registrations

If you are not currently registered with a practice in Crawley and will not be here for long, the Urgent Treatment Centre at Crawley Hospital is able to treat most minor illnesses and injuries.

If you are told that you need to register with a GP temporarily in Crawley, and you're staying within our catchment area, you can register temporarily with the practice. This means that you stay registered with your home GP surgery, but we can also see you and provide treatment while you are staying in Crawley. You cannot register with us as a temporary patient if you are already registered with another Crawley GP surgery.

You can only stay a temporary patient for up to three months. After this, you will need to apply to join the practice as a normal, permanent patient. This means that you will no longer be registered with your previous GP.

Please not that a GP practice does not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment.

Disabled Patient Facilities

Our consultation rooms are mostly on the ground floor and there is a ramp into the surgery. We can arrange for interpreters during appointments. Please let us know when booking if you'll need an interpreter or if you need a ground floor consultation room.
